Synthetic thyroid hormone drug, the levorotatory isomer of thyroxine. After partial conversion to triiodothyronine (in liver and kidneys) and entering the cells of the body it affects the development and growth of tissues and metabolism.
In small doses it has anabolic effect on protein and fat metabolism. In high doses it stimulates growth and development, increases the need for oxygen in tissues, stimulates the metabolism of proteins, fats and carbohydrates, increases the functional activity of the cardiovascular system and CNS. The therapeutic effect of L-Thyroxine is observed after 7-12 days, during the same time the effect is preserved after discontinuation of the drug. In case of hypothyroidism the clinical effect appears after 3-5 days. Diffuse goiter shrinks or disappears within 3-6 months.
L-Thyroxine Indications
- hypothyroidism;
- euthyroid goiter;
- as a replacement therapy and for prevention of goiter relapse after resection of the thyroid gland;
- thyroid cancer (after surgery);
- Graves' disease: after reaching the euthyroid state with thyrostatics (as monotherapy or combined therapy);
- as a diagnostic tool for the thyroid suppression test.
L-Thyroxine Contraindications
- hypersensitivity to the drug components.
- untreated thyrotoxicosis;
- acute myocardial infarction, acute myocarditis;
- untreated adrenal insufficiency.
The drug needs to be prescribed with caution in cases of ischemic heart disease (history of atherosclerosis, angina pectoris, myocardial infarction), arterial hypertension, arrhythmia, diabetes, severe long-standing hypothyroidism, malabsorption syndrome (may require dose adjustment).
L-Thyroxine Cautions
In case of hypothyroidism due to a problem involving the pituitary gland, it is necessary to find out whether there is a simultaneous adrenocortical insufficiency. In this case, the replacement therapy of GCS should begin before the start of treatment of hypothyroidism with thyroid hormones to prevent the development of acute adrenal insufficiency.
L-Thyroxine Effects on the ability to drive and use machines
The drug has no effect on the ability to drive vehicles and perform work requiring high concentration.
